If you are a new client, initial intake and consent forms will be emailed to you prior to your scheduled intake appointment.

Payment is accepted at the time of service. Checks and all major credit cards are accepted forms of payment.

Due to the complexities and time delays of insurance reimbursements, this office requires that each session be paid in full at the time of service. If a client wishes to utilize his or her health insurance for reimbursement, this office will provide the client with a superbill.

Therapy sessions are strictly confidential and will not be disclosed without the client's written permission. The State of California has certain laws that mandate a therapist to break confidentiality only under specific instances. Please refer to Soul Therapy & Coaching forms for more details about limitations to confidentiality.
